EOB Forms – EBF CO-PAY REIMBURSEMENTS

If you need to retrieve Explanation of Benefits (EOB) forms for the EBF co-pay reimbursements, go to the United Healthcare web site, www.myuhc.com , sign in (register if you don’t have a user name/password).  You may then access the EOB forms sent to you in calendar year 2015 to be submitted with your co-pay reimbursement form (deadline to submit is next Friday, 3/31/16).


SICK LEAVE BANK OPEN ENROLLMENT

Open enrollment for the CSEA-UCS Sick Leave Bank is from 3/24 through 4/20/16.  Current members need not reapply, and there is no contribution for 2016.  Those eligible to join the Bank are strongly encouraged to do so.  Application information has been sent to all employees from UCS HR personnel.

PHONE CALLS

We recently notified you that AFSCME/CSEA has been making phone calls to members’ homes for the purpose of providing updates on the Supreme Court Case of Friedrichs and updating membership information.  We encourage members to take two minutes of your time to take a call from AFSCME.

But there have also been reports of members being called by anti-union groups, trying to persuade the caller to quit their union. 

So if you happen to get a phone call, we would ask you to please pay careful attention to who is calling, write down the name of the organization, when the call was received, and let me know that information so that I can pass it along to CSEA Region 5 staff/LRSs.
